High-level disinfection traditionally is defined as complete elimination of all microorganisms in or on an instrument, except for small numbers of bacterial spores. In the past, high-level disinfection was recommended for mouthpieces and spirometry tubing (e.g., glutaraldehyde) but cleaning the interior surfaces of the spirometers was considered unnecessary. The U.S. Food and Drug Administration in September 2015, cleared sterilants and high level disinfectants with general claims for processing reusable medical and dental devices, peracetic acid for indication for device sterilization, glutaraldehyde and isopropanol for indication for device sterilization, hydrogen peroxide for no indication for device sterilization, ortho-phthaladehyde for no indication for device sterilization.

  • Ensure cleaning staff are trained on proper use of cleaning (and disinfecting, if applicable) products. Wash hands with soap and water for 20 seconds after cleaning. Be sure to wash your hands immediately after removing gloves. If hands are visibly dirty, always wash hands with soap and water. If soap and water are not available and hands are not visibly dirty, use an alcohol-based hand sanitizer that contains at least 60% alcohol, and wash with soap and water as soon as you can.

  • Critical Items- Critical items confer a high risk for infection if they are contaminated with any microorganism. This category includes surgical instruments, cardiac and urinary catheters, implants, and ultrasound probes used in sterile body cavities. Most of the items in this category should be purchased as sterile or be sterilized with steam if possible. Heat-sensitive objects can be treated with EtO, hydrogen peroxide gas plasma; or if other methods are unsuitable, by liquid chemical sterilants.
  • Semicritical Items- Semicritical items contact mucous membranes or nonintact skin. This category includes respiratory therapy and anesthesia equipment, some endoscopes, laryngoscope blades, esophageal manometry probes, cystoscopes, anorectal manometry catheters, and diaphragm fitting rings. These medical devices should be free from all microorganisms; however, small numbers of bacterial spores are permissible. Semicritical items minimally require high-level disinfection using chemical disinfectants. Glutaraldehyde, hydrogen peroxide, ortho-phthalaldehyde, and peracetic acid with hydrogen peroxide are cleared by the Food and Drug Administration (FDA) and are dependable high-level disinfectants.
  • Noncritical Items- Noncritical items are those that come in contact with intact skin but not mucous membranes. Intact skin acts as an effective barrier to most microorganisms; therefore, the sterility of items coming in contact with intact skin is “not critical”. Noncritcal environmental surfaces include bed rails, some food utensils, bedside tables, patient furniture, and floors. Noncritical environmental surfaces frequently touched by hand (e.g., bedside tables and bed rails) potentially could contribute to secondary transmission by contaminating hands of health-care workers or by contacting medical equipment that subsequently contacts patients.
